Insurance, Billing & Records:

 

Payment, including any co-payments or deductible amounts that you may have, is due at the time of service. Our charges vary depending on the complexity of your medical problems and the time required to diagnose and treat you according to strict criteria established by national standards.

 

Regardless of your insurance company, you must present your insurance card at the time of each visit.

 

A nominal fee may be charged for filing more than one insurance form, completing disability forms, statements to attorneys and insurers, and duplicating medical records.

 

We keep careful records of all visits, tests, etc. We will not release information or discuss your case with anyone, including family members, without your written permission.

 

Billing Questions and Contact

 

As you know if you have ever checked into a hotel or gone to a dentist, the first thing you are asked for is a credit/debit card, which is imprinted and later used to pay your bill. This is an advantage for both you and the hotel or dentist since it makes checkout easier, faster, and more efficient. At that time, any remaining balance owed by you will be charged to your credit card. A copy of the charge will be mailed to you, upon request.

 

Due to patient demand, we have implemented a similar policy. You will be asked for a credit/debit card number at the time you check in and the information will be held securely until your insurances have paid their portion and notified us of the amount of your share. At that time, any remaining balance owed by you will be charged to your credit card, and a copy of the charge will be mailed to you upon your request. This new process will be an advantage to you, since you will no longer have to write out and mail us checks. It will be an advantage to us as well, since it will greatly decrease our billing costs. The combination will benefit everybody in helping to keep the cost of healthcare down.

 

We require that you provide this information before your visit with the provider, so your cooperation in completing the form prior to arrival is appreciated. This in no way will compromise your ability to dispute a charge or question your insurance company’s determination of payment. We will charge you only the portion of the insurer-determined payment apportioned to you, the patient. Co-pays are still due at the time of the visit. This policy does not apply if you are a cash-paying patient. You will continue with the same policy that we have in place now and will pay at the time of each service.

Appointments and Scheduling

 

Please call for an appointment. Our phones are least busy during late morning and late afternoons. Please do your best to be on time for your appointment. In fairness to others, if you are late, you’ll be asked to wait for the next available opening. When making an appointment, let the receptionist know all the problems that need to be addressed so that we can be prepared to meet your needs in the time allotted to your visit.

 

Usually, you can be be seen the same day you call! If you wish to see a specific health care provider at a specific time, you may need to schedule in advance.

 

We provide urgent care appointments on Saturdays 9am to 2pm and Sunday 10 am to 12 noon at one of our offices. If you have a medical emergency, please call the office first to see if a doctor is here to care for you.

 

We try very hard to stay on schedule, but if unforeseen delays occur, we will let you know as soon as possible.

 

 

Prescription Refills

 

Our goal is to provide the best medical care possible to our patients and prevent any medical errors in prescriptions.

 

With your participation, your provider will decide on the frequency of visits needed to manage your medical condition and will also provide you with enough medication and refills until your next visit.

 

We suggest you make an appointment when you have one month’s supply remaining.
